Craving Change

This 3-part workshop will focus on exploring and creating a more positive relationship with food. Led by our Registered Dietitian and Registered Psychotherapist, various topics will be considered including understanding why we eat the way we do, food as comfort and comfort without food, and behavior patterns related to eating.

Our Registered Dietitian / Certified Diabetes Educator offers nutrition counselling for a variety of health issues including: diabetes, pre-diabetes, cholesterol, hypertension, weight management, gastrointestinal health, osteoporosis, fatty liver, anemia, GERD, gout, and metabolic syndrome.

Our two practitioners have training through the Heart and Stroke Foundation to provide assessment and ongoing support for hypertension management. We also have a 24-hour BP monitor for screening purposes.

Our Certified Diabetes Educators provide counselling and guidance to manage Type 1, Type 2 and Gestational Diabetes. We also offer counselling for pre-diabetes. We offer both individual and group counselling and family members are most welcome.

The chiropody service offers complete foot assessment, preventive education, and treatment for many conditions such as corns, calluses, warts, fungal infections, ingrown toenails, thick toenails, foot / toe deformity, heel / foot pain and foot ulcers.

Supportive counselling services are available by referral via our two Registered Psychotherapists to rostered patients with the West Champlain Family Health Team.
Memory Clinic*
Our team includes Kathy O'Connor, NP; Connie Daiken, RN; Darlene Denis-Friske, RP; Kristine Dewar, RP; Caitlyn Brydges, Alzheimer's Society representative, Dr. Lori Moeck and Dr. Fahamia Koudra.
Last Monday of each month
Our memory clinic is a comprehensive assessment of those individuals who may be having some memory impairment. A full cognitive assessment, including driving safety, is completed. Family support and resources are provided. Our goal is to help those with memory difficulties maintain independent living and the best quality of life.
Lung Health Program

Facilitated by our Registered Respiratory Therapist, our Lung Health Program provides education and support to individuals diagnosed with COPD and Asthma. Through this program, patients receive education and techniques to enable self-management and improve quality of life.

Dialectical Behavior Therapy (DBT) was originally developed by Dr. Marsha Linehan, and is considered to be an effective therapy in helping people manage overwhelming emotions. This is a 12-week group program requiring referral from your health care professional and an intake appointment.
The Mind and Body Fitness Program

This 8-week group program will offer a wellness focus with a different health team expert each week along with 40 minutes of moderate to high intensity exercise.
Moderate - High Mobility Excercise Class

Mondays & Wednesdays from 11:00am - 12:00 pm.
Seniors fitness classes are provided by our two certified Seniors Fitness Instructors. Classes are ongoing on Mondays and Wednesdays from 11:00 - 12:00 pm and are suitable for those who wish to maintain or improve their physical and functional fitness.
Low Mobility Excercise Class

Every Wednesday from 1:00-2:00pm, first floor
The low mobility class is suitable for those who wish to be more mobile, steady, and maintain their independence and functional abilities. Classes are ongoing, Wednesdays from 1:00 - 2:00 pm.
Functional Fitness

Thursdays from 10:00 am - 10:40 am.
The class incorporates functional exercises that will promote flexibility, strength building and balance which will help with activities of daily living.

This program is very popular because its fun! Based on education and exercise sessions to help seniors reduce the risk of falls and have more independent lifestyles.
Smoking Cessation (Quit Support)*

This program provides education, support and counselling to those individuals interested in quitting smoking or reducing the harm from smoking. Our two trained counsellors follow the Ottawa Model for Smoking Cessation guidelines. Options for treatment are detailed and free ‘Nicotine Replacement Therapy’ is available.
